DESCRIPTION:To schedule an appointment time to receive an Influenza vaccine or COVID-19 booster\, please click the link below: \nhttps://www.mtreadyclinic.org//appointment/en/reg/0769580621 \n  \nFLU AND COVID CLINIC REGISTRATION INFORMATION \nRegistration Links for each of the advertised FLU and COVID vaccine clinics can be found on the calendar under the “Events” tab at www.danielsmemorialhealthcare.org .  If you are unable to complete online registration you can call the DMHC Immunization office at 406-487-2313 and we will help you register over the phone. \nThings to note when registering online: \n\n\nOnly your insurance type will be requested.\n\n\n\n\nIf you are new to DMHC we ask that you bring your insurance card with you to your appointment\n\n\n\n\nSpouse or dependent can be added during the same registration if you have the same insurance type.\n\n\n\n\nThere is a section that will ask you to upload your vaccine records\, you can SKIP this part if you have been seen at DMHC before for immunizations.\n\n\n\n\nPlease be sure to complete each screen. You will be asked to review your information before saving and the last screen will say “Appointment Confirmed”.\n\n\n\n\nYou will receive an Appointment confirmation email. In that confirmation email you have the option to cancel your appointment at any time or reschedule to another open appointment time.\n\n\n\n\nPark in East parking lot designated areas for Flu shots.\n\n\n\n\nAppointments will be in the DMHC Trailer parked in front of the clinic on 5th Avenue.\n\n\n\n\nIf you are unable to come into the trailer\, we will be happy to come to your car\, please call us at 487-2313 for assistance.
