The $30.00 Sports Physical Exam is offered to all area students and will be provided at Daniels Memorial Healthcare Clinic. Please report to the CLINIC ENTRANCE between the hours of 3:00PM and 7:00PM with $30.00 and your complete, signed sports physical form. No appointment is necessary for the below dates.

Tuesday, July 25       3pm – 7pm

Tuesday, Aug 1          3pm – 7pm

Back to school immunizations are available during Sports Physicals. Parents can fill out consents and choose required or optional vaccines through the secure links below. Please call Carleen at 406-487-2313 for any questions!
